Visa Application Requirements

Essential Documents

Completed Visa Application Form

  • Can be filled in block letters or hand writing (must be hardcopy)
  • A recent photograph must be attached
  • Form must be signed and dated by the applicant
  • Download the official form here (See attachment below). In embassy/consulate they will give you form to fill out if you didn’t complete already

Passport-Size Photo

  • Size: 3×4 cm
  • Recent photograph
  • Clear white background

Passport and Copies

  • Valid passport
  • One copy of passport’s front page
  • One copy of Malaysian pass (if applicable)

Personal Letter from Applicant
Must include:

  • Planned date of entry
  • Purpose of visit
  • Transportation details
  • Hotel booking information (you can be book onward tickets from hotel.tiketvisa.com)
  • Detailed itinerary (See attachment below).

Supporting Documents from Sponsors (if applicable)

  • Marriage certificates
  • Birth certificates

Travel Documents

Proof of Financial Support
Either:

  • Latest bank statements (previous month)
  • OR front side copy of credit card

Single-Entry Visa:

  • US$16.00

Multiple-Entry Visa (up to 3 months):

  • B$30.00
  • US$24.00

Multiple-Entry Visa (more than 3 months):
